Tirzepatide vs Retatrutide: Mechanism and Trial Data

Reviewed September 6, 2026. Tirzepatide targets GIP and GLP-1 receptors; retatrutide adds glucagon-receptor activity. Both have clinical research records, but their regulatory status and the maturity of the evidence differ. A larger weight-change percentage from a separate study does not establish that one is superior.

What the additional glucagon receptor means

Retatrutide’s triple activity makes it a distinct molecule, not simply another concentration of tirzepatide. The discovery research describes its receptor pharmacology and early metabolic findings. Receptor counts alone cannot predict every clinical outcome, tolerability difference or experimental result. Original retatrutide discovery paper.

For laboratory comparisons, molecule identity, documented material form and analytical quality must be established before interpreting a difference between samples. A catalogue name or nominal vial amount does not establish those properties.

What the weight-loss studies actually show

In SURMOUNT-1, 2,539 adults with obesity or overweight and a weight-related complication, without diabetes, were randomized to tirzepatide or placebo. At 72 weeks, the 15 mg study arm showed mean weight change of −20.9%, versus −3.1% with placebo. Gastrointestinal events were common. Jastreboff et al., NEJM (2022).

Retatrutide’s Phase 2 obesity trial followed a different population for 48 weeks. Its often-quoted 24.2% result came from the 12 mg study arm. Retatrutide Phase 2 publication. The studies differ in size, follow-up and design; subtracting the two percentages does not estimate a head-to-head treatment advantage.

Has retatrutide been compared directly with tirzepatide?

Yes, a direct comparison is underway in TRIUMPH-5. As of the registry check for this review, no results were posted and primary completion was estimated for November 2026. The correct distinction is between an ongoing comparative trial and a completed published result. TRIUMPH-5 study record.

Until results are available, this page does not label either compound the clinical winner. Future evidence should be assessed using the trial’s prespecified endpoints, uncertainty estimates and adverse events, not only its largest reported weight change.

Safety findings are part of the comparison

Both programs report gastrointestinal adverse events. Tolerability, discontinuation, serious events and the populations excluded from a trial all affect interpretation. Separate trials may use different collection methods and follow-up, so their adverse-event percentages are not a controlled comparison either.

Are tirzepatide and retatrutide approved in Canada?
